I get something similar sometimes but its just my controls have been reset.
i don't know why it does this. maybe should keep a back up of your user file in your system folder.

What was the name of the file?

Without knowing that, the only thing I can suggest is a reinstall of UT or Windows.

Well...if you look within your UT2004/System folder, you'll find a file called DefUser.ini. You can either rename that file to "user.ini" or just copy all the info within it..and paste over all the info that is in the user.ini file. The DefUser.ini file will reset all user info...such as player model, controls, sound settings, display settings etc. so you'll have to go through and change all your custom settings again. You can always make a backup copy of your original user.ini file just in case you want to restore the settings that you had prior to the change.

If using the default info in the user.ini file doesn't help your problem...then you're issue is going to be somewhere else. It's possible that you have a custom mod or mutator installed on your game that is causing issues, keyboard driver issue, the keyboard itself, or possibly even the port that your keyboard is plugged into. If it isn't within the game itself...then it's obviously a hardware/system/software problem :D
